Required documents
If you plan to apply for a Skills Assessment, there are certain required documents that you must submit with your application.
Please note:
- applications missing the required documents will be delayed.
- we will contact you for additional documentation, if required.
- documents will not be returned.

| Application form | Download the SRG1 Application for Skills Assessment Form should be completed and signed. |
| 2 recent photographs | Must be passport sized, certified and dated. |
| Birth certificate or passport | Must be certified (passport page must show applicant name, photo and date of birth). |
| Change of name | If applicable. |
| Qualification Award | Must be certified. If this is unavailable a statement of completion from the academic registrar may be accepted in lieu. Read additional requirements below for individual countries. |
| Transcript | Must be certified and show subjects studied and marks/grades awarded in the original language. |
| Employment evidence | Must be certified or original documents. Read employment evidence for more details of acceptable documentation. 485 visa applicants are not required to provide employment evidence. |

| Certified English translations of all documents | Should use a registered translation service, if documents have been issued in a language other than English. |

Additional requirements
| Europe | Please include ‘Diploma Supplement’ if one has been issued. Most European universities have issued a Diploma Supplement automatically and free of charge to graduates from 2005. Supplying a Diploma Supplement provides a precise description of the applicant’s academic study. |
| Nigeria | A verified copy of the award certificate and full academic transcript sent directly in a sealed envelope from the awarding body to VETASSESS. |
| Pakistan | A verified copy of the award certificate and full academic transcript sent directly in a sealed envelope from the awarding body to VETASSESS. |
| People’s Republic of China | Qualifications awarded in China must be verified by Qualifications Verification at VETASSESS. Please submit your qualification documents as outlined above. Note: Qualification documents do not need to be notarised or certified. Please click here for more information |
| Philippines | Evidence of membership of a professional board in the relevant professional field may affect the outcome of an assessment of qualifications from the Philippines. Applicants from the Philippines are asked to submit evidence of passing the Professional Regulation Commission’s Licensure Examinations in the relevant professional field as this may positively affect the assessment outcome. |
